The Adolph and Rose Levis Apartment Program
- Designed for adults who do not require 24 hour supervision in order to lead fulfilling lives
- Spacious two bedroom apartments with common living/dining areas
- Supported living coach assists clients with skills such as budgeting/money management, paying bills, grocery shopping/meal preparation, medical, transportation
- All clients receive individualized programs based on their needs
- Participation in organized leisure skills, yet enough flexibility to plan their own activities with friends
- Residential manager on staff
Supported Living Admission Criteria
- Be 21 years of age or older
- Desire an independent lifestyle
- Be capable of living and sleeping alone without staff supervision present in the apartment
- Can identify emergencies, distinguishing between emergency and non-emergency situations and is capable of calling for help
- Upon admission, pose no threat to self or others, displays no loud or disruptive behavior
- Performs all activities of daily living. If prevented by a physical disability, provides a personal aide to accommodate the need
- Demonstrates reasonable precautions regarding personal safety (locking doors and windows, not letting in and talking to strangers or hitchhiking, etc.)
- Follows apartment safety rules regarding electrical outlets and appliances
- Follows medical orders, including taking mediation without staff
- Responsible for preparing all meals for him/herself
